Portuguese term

Clínica

COVID-19 Portuguese to English Medical Medical (general) Secondary illnesses suffered by Covid patients
I know this term has been much discussed before but I would like some advice on it in this context if poss. please!

"Sem clínica focalizadora"
"Clínica de febre, cefaleia e alteração do estado de consciência."
"Tendo em conta os aspetos descritos, a clínica, e os antecedentes de instrumentação uterina..."

I know the word can mean therapy/treatment, application/use and clinical practice etc. but don't know what best suits here.

These are doctor's notes so I don't have very much context outside of the phrases themselves.

Any ideas much appreciated, thank you!

Medical condition

Clíca de febre, cefaleia describes the "quadro clínico" which would be the medical condition.

Conjunto formado pelos sintomas, sinais e pela evolução de uma doença (ex.: a paciente não apresenta clínica de parkinsonismo).

"clínica", in Dicionário Priberam da Língua Portuguesa [em linha], 2008-2021, https://dicionario.priberam.org/clínica [consultado em 05-12-2021].

A medical condition is a broad term that includes all diseases, lesions, and disorders.
While the term medical condition generally includes mental illnesses, in some contexts the term is used specifically to denote any illness, injury, or disease except for mental illnesses. The Diagnostic and Statistical Manual of Mental Disorders (DSM) uses the term "general medical condition" to refer to all diseases, illnesses, and injuries except for mental disorders.
As it is more value-neutral than terms like disease, people sometimes prefer the term "medical condition."
The term medical condition is also a synonym for medical state, which describes an individual patient's current state from a medical standpoint.

clinic; exam/test/direct observation(s)

I don't believe there's a one-size-fits-all translation for the three phrases that were provided. This is how I would translate the first one:

sem clínica focalizadora: without focal clinic
https://www.sciencedirect.com/science/article/pii/S0264410X1...

As far as the other two, the translation is a little more problematic since we don't know whether "clinic" refers to exams/tests or simply medical observations.

According to Aulete Digital:

clínica: A observação direta, pelo médico, do estado do paciente, incluindo exames mais ou menos simples (auscultação, medição de temperatura, etc.), ou outros, além de perguntas etc., e guiada pela combinação de conhecimento, experiência e sensibilidade: "A clínica é soberana, isso quer dizer que nenhum médico, em sã consciência, pode fazer o diagnóstico de um paciente sem vê-lo, baseado apenas em um resultado de exame de laboratório." https://www.aulete.com.br/clínica

Apparently, "clinic" does not translate "clínica" as defined above. See https://www.merriam-webster.com/dictionary/clinic

So for the second and third phrases, I suggest slashing the translation, something like this: exam/test/direct observation. We don't know exactly what they are referring to. Another option is to ask the client for clarification. Good luck.

PS: If for some reason I don't respond to comments and/or disagrees, it's either because I'm too busy or simply chose not to address any negative/disagreeing comments that I've been getting for no apparent reason. My refusal to engage with whoever thumbs me down doesn't mean that I agree with them. Asker, please feel free to contact me (via inbox) if you need any clarification about my answer. Thank you.

here, used as a noun - the abbreviated form of clinical examination
Peer comment(s):

neutral Oliver Simões : "Clinical" wouldn't fit any of the phrases since it's not a noun, it's an adjective! // Katarina, "clínica" is being used as a noun in all three examples.
