Antes o durante El doctor describe el procedimiento inicial: Se coloca paciente en decubito lateral izquierdo y en tractor de miembro superior derecho. Se realiza portal subacromial, etc.

No mencionas si el paciente se coloca 'en tractor' antes, durante o después de la cirugía. Si se refiere a después de la intervención, se me ocurre 'sling', 'brace' e incluso 'pulley system'.

I think you need to reword it.

The patient is placed in the lateral decubitus position with their right upper limb held in a traction system/supported by a traction system.

The skin traction construct is attached to a rope. The rope is passed over the upper arm of the C-arm apparatus. The traction construct is wrapped by a sterile towel secured with sterile gauze bandages to avoid contamination.

Intraoperatively, a longitudinal, perpendicular traction device is used to hold the arm in an abducted position (Figure 2, B). The appropriate amount of abduction (combined with varying degrees of flexion) required for visualization remains a topic of debate and ranges from zero to 90. Klein et al12 reported a reduced incidence of brachial plexus strain when the arm was positioned at 45 of forward flexion and both zero and 90 of abduction.
